NY Producer Compensation Disclosure (Reg 194)

Silverline Insurance Agency (“Producer”) is licensed as an insurance producer and broker by the State of New York.

Producer’s role in this transaction:

Producer can act as insurance broker or insurance agent or  insurance producer in connection with the sale of this insurance contract.   The role will be clarified when insurance products are identified and sold.   

Compensation:

Producer will receive compensation from the insurer issuing the insurance contract and/or from another third party, based in whole or in part on the insurance contract sold.

Compensation may vary:

The compensation paid to Producer may vary depending on a number of factors, which may include the insurance contract and insurer selected, the volume of business Producer provides to an insurer, and/or the profitability of the insurance contracts Producer provides to an insurer.

Right to request more information:

You may request information about (i) the compensation expected to be received by Producer based in whole or in part on the sale, and (ii) the compensation expected to be received based in whole or in part on any alternative quotes presented by Producer.
